Family Child Care is not Babysitting. Its not comparable to any child care center. Its Families in Relationships with a Professional care provider in an environment that becomes, By Design a second home, and a whole Bunch of People partnering, trusting, caring and education each other.
This agreement terminates in one year from your contracts date. Failure to comply with the terms set forth in this agreement may, at Provider’s discretion, result in immediate termination of enrollment and forfeiture of any security deposit paid. A two week written notice is required for any party to terminate this agreement, after the trial
adjustment period has expired. If you terminate the without two week notice, you will still be held liable for two weeks fees/forfeit deposit.
The payment rate will be due on the 1 st (Monday) of each week of care. You are paying for a position, as well as a service, Payment is flat weekly rate. You were given your rate at time of contract. Provider will give the parent/guardian a minimum of two week notice of any increase in fees. Provider does not have hourly or daily
rates. Form of payments: Cash, Money Order, Credit Card and Cash App (available upon request). You are responsible for any fees occurred in credit card charges ($1.00 charge for credit cards) * to avoid late fee if
Holiday falls on Monday Cash payments should be made before Holiday not after. Unless otherwise noted in writing or another form of communication Provider does not pro-rate or give refunds due to absence in care for any reason. When paying with cash please provide correct amount, as provider does not give change back, but will credit your child/ren’s account for the following week.
Fees are payable in advance, no exceptions are made for absences due to illness, vacation, or any other reasons for
missed attendance. Payment is due on the 1 st (Monday) of the week. There are no refunds for late arrivals or early pickup. Payment is a flat fee. If child is absent from care on a scheduled day the normal rate will still be due. If you need additional hours beyond agreed upon contracted pick up & drop off times, a rate of $10.00 per hr. will be 11 added to your rate on such days additional time is needed. This must be pre-arranged and approved by provider in advance.
Late fees will be charged at the rate of $5.00 per 15 minutes and anything in between past contracted pick up time. This also applies to early drop off. Provider also reserved the right to refuse a child if drop off occurs before contracted time. Repeated early drop off or late pick up may result in termination of care. Late fees are due at
time of pick up or by next morning. Please remember provider has commitments to her family and others outside the home and may not always be available for late pick up. Please have someone else available to pick up your child for you if you will be late.
I DO NOT ACCEPT CHECKS AT THIS TIME. In the event of a returned or NSF check parent/guardian will be responsible for all bank charges incurred by provider and a $__ NSF fee will be charged to your account. If payment is not received within 24 hours of NSF notification normal late fees will be accrue. Failure to pay within 5 days will result in termination of contract for non-payment and forfeiture of any deposit. After two NSF checks, all future payments must be paid in cash or money order only; checks will no longer be accepted.
Provider opens and closes when your child arrives and is picked up. Provider does not carry general open and close hours, each family has their own contracted drop off and pick up hours. These hours are strictly enforced. Repeated early drop off or late pick up (outside of contracted hours) may result in additional fees and/or immediate termination of care. If you occasionally need additional hours beyond agreed contracted hours, a rate of $10.00 per hr. will be added to your rate on such days additional time is needed. This must be pre- arranged and approved in advance. ( No child should be left in care for no more than 9.5hrs per day)
The Parties will agree to a contracted schedule of care. Provider opens and closes when your child arrives and is picked up, she does not carry general open & close hours. Each family has their specific and pre-arranged drop off and pick up hours. Additional days will depend on availability and will be charged at a drop in rate.
Provider will be closed with pay on Listed Holidays, development training days & Vacation. See Holiday & Vacation closing days insert sheet. Parents will be reminded of closures prior and notified of any additional closures dates surrounding Christmas. VACATION The Provider will take (2) 5day vacations during the year.
*See Holiday & Vacation Closure list for dates. Once your child has been in care for one year you are eligible
to take a 2week vacation without payment. Your Vacations may be split into 1/1 week but not into individual days. You are not subject to taking/or using vacations weeks same as the provider however, If vacation days are not taken on same weeks as the provider but are used on different weeks other than, tuition will still be due when the provider vacation days are used. If your child has not been in care for a year you are still responsible for your childcare tuition during vacation times. Please notify provider two week in advance of vacation dates. The use of 13 “free and ½ rate Vacation times cannot be used in conjunction with a Holiday or any schedule breaks. Please note if you pull your children from care and they return, your time (year) will start when they become re-enrolled.
When choosing Family Child Care it is imperative that you have reliable back up care available. In the event of provider illness, vacations, or personal time off, training or family emergencies, it is the responsibility of the client to arrange alternate care. However, I am very reliable and hardworking, so I will make the utmost effort to remain open when at all possible.
Please dress your child/ren appropriate for the weather outside. In the event that weather creates a dangerous situation, you may be asked to pick up your child up from care immediately or I may be forced to close for the day entirely. Examples of this may be, but are not limited to, loss of electricity or threat of serious flooding. I will notify
parents immediately of any weather related closures. In the Unforeseen weather related closure please note that full payment is still due. Provider does not pro-rate in cases as such. Parent/guardians are responsible to have back-up care available in case of these closings. Please be sure to stay connected on our “Parents only Facebook Page”
In the event that your child needs to miss a day, please call within 1hour of regular arrival time. Please remember even if your child is absent from care the normal rate will still be due. You are paying for a position, as well as a service. If your child does not arrive within one hour of regular drop off time, with no phone call from parent/guardian and proper documentation, I will assume your child will not be in care for that day and may make other plans for the day. It will then be up to you to find care for the day if needed.
A registration fee of $50 will be due at the time of registration. Handbook and application will not be given until fee is paid. There will be a $25 supply fee billed twice a year. These fees are used for the purpose of getting supplies but are not limited to tissue, Kleenex, wipes, soap, cleaning supplies, paper, ink, equipment, toys, and materials. Occasionally parents may be requested to pay extra fees for field trips and/or other special projects and activities. Parents will be notified in advance of any special projects or field trip. You may choose not to participate in special fields’ trips or activities that require additional fees; however alternate care may need to be arranged by parent/guardian. Hold Spot- Holding spot in our care for expecting families, will be a contracted monthly fee given at time of registration until start date. This fee can be paid at once or monthly and is non- refundable and does not go towards in future payments. This fee will guarantee your spot is held until your start date.
Under no circumstance should a parent bring a child to care sick. If you are not sure if your child is well enough to attend care please call and discuss it with provider. Masking your child’s symptoms with over-
the –counter medications and bringing them to care anyway is not allowed and may be caused for immediate termination. Provider understands and respects your need to work, but is responsible for her health and the health and well-being of all children in care and will closely follow health department regulations when it comes to illness.
For the health and safety of all children the provider has the right to refuse to care to any sick child. If your child has been diagnosed with an illness is your responsibility to let the provider know. IE. Provider can and may require extra days of absence from childcare beyond what you Pediatrician has suggested depending of level of sickness.
Nonprescription and prescription medications, ointments, and creams can be given to your child if needed. Parents are required to fill out the proper forms and to supply all medications in their original containers. Vaccinations – It is strongly recommended & encouraged that your schedule your child’s vaccine appointments on
Fridays. Occasionally children may have mild to serious side effects due to vaccines given. I.e. fevers, rash, nausea, or pain etc. If shots are schedule during the week we recommend that your child is cared for under parent/guardian supervision for at least 24hrs before returning to care.
Minor bumps and scratches are inevitable, but I make every effort to keep children safe through supervision and
childproofing. Minor injuries receive appropriate first aid and if an emergency injury or illness occurs, you will be contacted as soon as possible. If necessary your child will be taken to the nearest hospital where you will be asked to meet us. Parent/Guardian are responsible for all cost involved in emergency medical treatment, including emergency transportation if required. Provider and/or her family will not be held liable for any sickness/injury of either parent/guardian or child while on the premises or while the child is in the company of the provider,
including during field trips or outings. The Provider does not carry Child Care insurance at this time.
It is normal for children to be hesitant and sometimes even cry when being dropped off. Please be very brief (no
more than 5 minutes is sufficient) during drop off times, as the longer you prolong the departure the harder it gets for your child. A smile, cheerful good-bye kiss, and a reassuring word that you will be back is all that is needed. Also please do not allow your child to come in/outside unattended. Pick up and drop off can be a time of testing limits when two different authority figures are present ( parent and provider) and children may test to see if the rules still apply. It is important that you 16 support provider’s rules at this time, this promotes consistency, and if
you do not provider will be remind your child of inappropriate behaviors being displayed and take action to correct them if needed.
Under no circumstances will a child be released to
anyone other than parent/guardian or those listed below without written permission or told by parent/guardian to provider. Even with permission, any person unfamiliar to provider will be required to show valid photo identification. Please alert provider in advance when anyone listed will be picking up; please also remember to inform the listed
named person(s) that a photo ID must be shown before child can be released to them.
No child will be hit, spanked, belittled, or otherwise
intimated while in provider’s care. No corporal punishment will be used, including by parent/guardian when on the premise. Children will be treated with courtesy, respect, and patience. Children are explained the rules of the childcare home frequently so that all know the guidelines.
Once a child understands the rules and disobeys them the following guidance techniques will be used according to age and understanding level. Younger children, older infants and toddlers, will be redirected to another activity. Older children will be directed to the “Thinking Chair” and can return to the group when they are ready. If a behavior becomes persistent, I will address the behavior with you and we will try and
resolve it together.
Be Kind. Be fair. Be safe. Be Neat. The following rules are reinforced for the safety and wellbeing of everyone. There is no running throughout the house. There is also no hitting, pushing, biting, grabbing,
kicking, or pinching of other children. Obscene language is not allowed. Children are not allowed to walk around the house with cups, bottles or
pacifiers . Respectful treatment of other children and all property, toys and furniture is expected. No smoking allowed on premises.
– Parents are required to notify provider within one hour of normal arrival time if
child will not be coming for the day.
– Parents will not bring their child(ren) to daycare after 9:00am unless with
Doc./Appt. Excuse or Unless later time is stated in Contract.
– Parents are required to keep me informed of any change in addresses,
telephone numbers, up dated shots and other pertinent information.
– Provider will promptly report to the proper authorities any sign of neglect or
abuse of the children in care.
– Provider’s household includes these pets: No Pets.
– Provider maintains an open door policy for parents during daycare hours. Please
avoid visiting during rest/nap time. Please be aware that the door is often locked
during the day for the safety of the children.
–Parents are welcome to call/text during the day or evening with any questions.
However provider’s cell phone is generally off between 7:30pm & 4:30am if
unable to reach provider, please leave a message and your call/text will be
returned as soon as possible.
–Willful destruction of property may be charged to the parent/guardian at the
cost to replace the item.
– Play is an important part of development and children will get messy/ dirty
during activities. Self –Feeding for Toddlers/Preschool can be messy as well.
Please do not send children in special outfits you do not want dirty or stained.
Parents of Toddlers and Preschoolers are requested to bring a blanket for rest time to be kept at the childcare home and sent home on Fridays. A complete change of clothing (including socks) appropriate for weather. Soiled clothing will be sent home and a clean change of clothes should be brought back the next day. Good/special clothing is not recommended. All items need to be labeled with your child’s initials. *Parents are required to supply diapers and wipes for non-potty trained 18 children. Provider will notify you when your child’s supply needs to be replenished. Parent should also bring diaper rash ointment, any medication, or cream (including sunscreen) as needed. * Please see Medication clause. Children on special diets or children who cannot eat what is on the menu must bring a nutritionally packed lunch.
No toys should be brought from home. I am not responsible for lost items. If it is something to sleep with during nap time (Special blanket) please bring it, but it will only be for naptime and will be put up
when the child first arrives.
Provider will be happy to help with “Potty” training
provided that parents initiate the process at home first, whether it is over a weekend or during vacation. Provider expects parents to work with her in the process. Putting a child in a diaper during the potty
training process will only confuse the child and delay the training process. All potty training children must wear clothing that they can handle successfully on their own- no onesies, no overalls, no belts, or
jeans with buttons or snaps. Elastic waist pants (Sweat pants) are the most appropriate. Parents will be asked to supply extra sets of spare clothing during the training period.
Parent/guardian(s) are welcome to bring special food
treats for their child’s birthday. Please arrange with provider beforehand as other children have allergies to specific foods. Children will learn about most major Holidays and provider may have holiday parties and or holiday related activities. If you’d prefer your child not participate in any planned parties, alternate care will need to be arranged during those times.
Breakfast, lunch, and snacks will be provided daily at no additional charge for full time clients. Parent/guardians are responsible for feeding your child/ren if he or she will arrive after a mealtime before entering childcare. Outside food is not allowed. Infants are always fed on demand. Infant formula & breast milk (bottles must be pre made and labeled prior to arrival), as well as cereal, and any other foods. Baby food will be provided by provider at appropriate age.
Each child is required to have a rest period. If your child no longer naps, he/she may look at books or do quiet activities but must remain quiet for the other children. Parents are requested to supply a blanket for the child to be kept a daycare during the week and returned on Fridays for cleaning. Drop off is not allowed during this period as it has proven to be a very disruptive to the sleeping children. Pick up
during this time must be arranged with provider beforehand to prevent disruption of sleep i.e. knocking on door, ringing doorbell or having to
wake a sleepy child. Infants are always put to sleep on their backs and nap on demand. Toddlers will be transitioned on cot to one nap a day usually between 12-18 months.
Provider may offer field trips at various times throughout the year. Notice of field trips will usually be given at least two weeks in advance. Parent will be required to find alternate care for the child if he/she cannot attend. Regular childcare fees will still apply for field trips days, whether or not your child attends. Provider will generally cover the cost of most field trips. Occasionally minor additional fees will be
requested for field trip activities, you will always be notified in advance if any additional fees will be required.
Occasionally provider may need to transport children in care for field trips or other activities outside childcare home. All traffic 20 laws will be followed and no children will ever be left unattended in a
vehicle. Children will be placed in age/weight appropriate car seats are required by state law. Parents may be requested to provide a car seat
for the day. Copies of emergency forms remain with provider at all times.
The first week of care is an adjustment and
trial period for the provider, child and parent/guardian. This agreement may be terminated, in writing, at any time during this period by the
parent/guardian or provider.
After the trial period, this agreement may be terminated by the parent/guardian by giving two weeks written notice if the child or children are to be permanently withdrawn from care. You forfeit any
and all amount of your two week down payment made. Your two week down payment will be applied to your child’s/children last two weeks of
care. Provider will also give the family two weeks written notice of intent to cancel this agreement except in cases of family emergency (Provider’s), gross misconduct on the part of the parent/guardian or child, and/or failure to follow the rules set forth in this contract, in which termination with no notice may be given.
All information provided will be kept confidential.
Absolutely no information will be released to persons other than parent/guardian unless prior written permission has been received.
I/we have fully read and understand this contract. By signing this agreement, I/We agree to comply with all
terms and provisions stated in contract and parent handbook. This contract is subject to review and renewal on the anniversary of your contract. Any changes made to the terms of this contract must be made 21 in writing by the provider and with two weeks’ notice to parent/guardian. Otherwise, this contract will remain in effect until the renewal date or upon termination of care as set forth herein. Both
parties agree to cooperate and work together on behalf of the child and accept this agreement as a binding contract.
Getting your child to school in a timely manner everyday, making sure they are ready for the day. Drop offs can be hard make them quick and do not linger reassure them that you will be back to get them at the end of the day. Having your child secured in the appropriate seat for the ride.
